Nix Senior SolutionsBy offering a complete range of medical services to meet the specific needs of the senior population, Nix Senior Solutions® is the answer to promoting senior wellness and managing your care or that of a loved one. At Nix, you can visit with your physician, see a medical specialist, such as a cardiologist or surgeon, obtain an x-ray, mammogram, and MRI and have lab work done – all without leaving the building.

Plus, we offer:
  • A distinguished team of board certified physicians* in family medicine, geriatric care and psychiatry
  • Direct admission into your private room, if a hospital stay is necessary
  • Acute Care for the Elderly (ACE) Program, which utilizes an interdisciplinary team, to help patients return to a productive, meaningful life as quickly as possible after a hospital stay
  • Inpatient Geriatric Behavioral Health program to meet the mental health needs for senior adults
  • Information, education and support for families and caregivers
  • Assistance with medication management
  • A complete continuum of care with physicians able to see you where you are, whether at our office, the hospital, a skilled nursing home, assisted living or long term care facility
  • Home care services to provide medical care in the comfort of the patients’ home
  • Private care services to assist with activities of daily living, transportation, as well as light housekeeping, laundry and other similar duties

At Nix Health, we are in close contact with a variety of medical specialists who have offices in the Nix Medical Center and/or Nix Alamo Heights. These established relationships allow for immediate appointments to specialists or specialized treatments. Plus, these relationships greatly assist us in truly coordinating our patients' medical care.

Our staff is familiar with the issues that face older adults and their caregivers and work closely with a trained Patient Navigator when necessary. Medicare changes are a constant battle for our patients. We work to help you navigate through the red tape and get the care you need and deserve.

SERVICES: NIX SENIOR HEALTH CENTER
The Nix Senior Health Center is an exclusive healthcare service of Nix Health, specializing in Geriatrics. Physicians are board certified in Family Medicine and medically trained to manage the medical care of elder adults.

Everyone in our office from the nurses to the office staff to the physicians, are individuals who have dedicated themselves to putting your health, or that of a loved one, first.

The Nix Senior Health Center provides the following services for our patients:
  • Complete evaluation of the senior adult
  • Managing chronic diseases, such as heart disease, diabetes, high blood pressure and high cholesterol
  • Coordinating care among specialists
  • Addressing the psychological, social and spiritual needs related to a serious or life-threatening disease
  • Managing medications, checking for harmful interactions and explaining side effects
  • Providing adult immunizations
  • Diagnosing and caring for those with:

    • Arthritis and/or Osteoporosis
    • Falls and walking difficulty
    • Hearing and vision difficulty
    • Incontinence
    • Memory disorders, dementia and Alzheimer's Disease

Our goal is to keep you healthy, independent and free to enjoy the quality of life that you deserve. We understand the special needs of dealing with multiple medical conditions and take the time to explain, in detail, prescriptions and side effects to you and your family and loved ones who might need to know.

In addition to caring for our patients in the hospital, our doctors and staff care for patients in our office and onsite at selected nursing homes and assisted living facilities.

When our patients need to be admitted to the hospital, we care for them at the downtown Nix Medical Center. The hospital staff is specially trained to care for the health concerns of the elderly.

Should you see a Geriatrician?

Generally, adults 55 years of age and older make up the majority of our patients. However, any adult who suffers from any of the following medical problems should consult with a Geriatrician:
  • Decline in being able to function independently
  • Repeated falls
  • Older adults with a disability or illness, even in the early stages
  • Older adults with complex medical and social problems
  • Caregiver issues
  • Dementia or memory loss
  • Confusion

With a focus on maintaining a healthy lifestyle, independence and disease prevention, patients and their caregivers are encouraged to utilize the healthcare services provided by our physicians*, who specialize in the care of older adults.

SERVICES: ACUTE CARE FOR THE ELDERLY (ACE) PROGRAM
The ACE Program at Nix Health is specifically designed to provide interdisciplinary care to hospitalized patients age 65 and older. Studies have shown that ACE programs reduce the average length of a hospital stay, better preserve a patient’s ability to function normally and increase patient and staff satisfaction.

An interdisciplinary team, lead by board certified physicians, consists of geriatric trained nurses, pharmacists, dieticians, physical therapists, case managers and a chaplain. This team is involved in patient care from the beginning of admission and meets on a daily basis to review patient’s progress and implement a personalized treatment plan for each patient.

With an emphasis on education and support for families and caregivers, the ACE Program is tailored to address the complex medical conditions of the senior and minimize the healing process of the patient.
